Prepare Basic Documents

Have these ready to support your claim:

Basic Requirements (All Claims)
  • Duly accomplished Claim Form
  • Written notice of loss / incident report
  • Policy number or copy of insurance policy / certificate
  • Valid government-issued ID of the insured / claimant
  • Proof of ownership or insurable interest
  • Detailed description of the incident (date & time of loss, location of loss, cause and circumstances, estimated damage amount)
For Motor Insurance
  • Repair Estimate — itemized list of parts to be repaired or replaced with costs, plus the estimator’s contact details (preferably a mobile number)
  • Photos — vehicle showing plate number and visible damages in one frame, plus actual photos of the accident scene / place of incident
  • Police Report and/or Incident Report (PhilFirst incident report form)
  • Official Receipt (OR) / Certificate of Registration (CR) — clear copy of the updated OR; engine and chassis numbers clearly readable on the CR
  • Driver’s License — clear, readable copy of the front and back, plus the OR of the license held by the driver at the time of the accident
