Prepare Basic Documents

Basic Requirements (All Claims)
- Duly accomplished Claim Form
- Written notice of loss / incident report
- Policy number or copy of insurance policy / certificate
- Valid government-issued ID of the insured / claimant
- Proof of ownership or insurable interest
- Detailed description of the incident (date & time of loss, location of loss, cause and circumstances, estimated damage amount)
For Motor Insurance
- Repair Estimate — itemized list of parts to be repaired or replaced with costs, plus the estimator’s contact details (preferably a mobile number)
- Photos — vehicle showing plate number and visible damages in one frame, plus actual photos of the accident scene / place of incident
- Police Report and/or Incident Report (PhilFirst incident report form)
- Official Receipt (OR) / Certificate of Registration (CR) — clear copy of the updated OR; engine and chassis numbers clearly readable on the CR
- Driver’s License — clear, readable copy of the front and back, plus the OR of the license held by the driver at the time of the accident
For Property / Fire Insurance
- Fire or incident report
- Photographs of damaged property
- Repair estimate or contractor’s quotation
- Inventory of damaged items
- Purchase receipts, invoices, or proof of value
- Barangay or police report (if applicable)
For Engineering Insurance
- Photographs and videos showing damaged property / equipment
- Repair or replacement quotations
- Purchase invoice or proof of ownership / value

- Equipment specifications and technical manuals (if required)

For Burglary / Robbery
- Police report
- Inventory of stolen items
- Proof of ownership and value
- Photos of forced entry or damaged premises
For Personal Accident
- Medical certificate
- Hospital records and bills
- Official receipts of expenses
- Police report (for accidents requiring investigation)
- Death certificate and beneficiary documents (for death claims)
For Marine Cargo Insurance
- Bill of Lading / Airway Bill
- Packing list and commercial invoice
- Delivery receipt
- Survey report
- Notice of loss or damage from carrier
For CG Liability
- Demand letter from third party
- Incident report

- Formal Claim / Demand Letter addressed to the surety company
- Copy of the Bond &/or Indemnity Agreement (surety bond and attached riders, if any)
- Copy of the Principal Contract / Agreement secured by the bond
- Proof of Default or Breach by the principal
- Statement of Claim / Loss Computation showing the amount being claimed
- Supporting correspondence between obligee and principal regarding the default
- Notice of Default / Termination served on the principal (if applicable)
